Prior to attending your first appointment, please print and complete the Client Information and Informed Consent forms and bring them to your appointment. It is important that you fully understand the nature of the therapeutic relationship and that you review the informed consent prior to attending your first appointment. Additionally, completing the following paperwork in advance will allow us to have more time during the initial intake consultation. At that time, we will review the paperwork and I will answer any questions that you have.
